Appointment Scheduling
We are currently scheduling new patient appointments and follow-up appointments.
New Patients:
- Referral Required by our Providers: Your primary care doctor must send us your referral and medical records before scheduling an appointment. Please refer to the “New Patients” tab for more details. We will contact you to schedule once all necessary information is received.
- Primary Care Requirement: You must have a primary care doctor to be eligible for a new patient appointment.
- To Schedule: Call us at (303) 321-2644.
Existing Patients:
- Appointments: If you need to make, reschedule, or cancel an appointment, please call our office at (303) 321-2644. Alternatively, you can log into your Follow My Health account to submit a request to change your appointment.
Insurance Information:
- Insurance Notice: Please note that we are not providers for Tricare, Medicaid, or Humana.
- Insurance Responsibility: All patients are responsible for contacting their insurance provider to confirm whether we are in-network with their policy.
